Update on Risk Management Forms

To our staff and faculty community,

With the restructuring of Risk Management and Safety Services, we have centralized the location where all Risk Management forms should be submitted for processing. As of today, the following forms will need to be submitted to risk-management@humboldt.edu for approval:

- Natural Area Entry Permits

- Volunteer Forms

- Alcohol Permits

- Insurance Certification Requests

There will be ongoing process improvements as we move forward. The first update is a revised Hazardous Waste pick-up request process. We will be phasing out the paper carbon copy “Transfer Tags” and will now be using a Hazardous Waste Pickup Request (Google) Form. The Google form for requesting pickups will help us provide faster service, avoid mail delays or losses and track completion more efficiently(bulk pickups from centralized stockrooms are excluded from this process).
