The CSU annual Open Enrollment period will occur September 14, 2015 through October 9, 2015. Open Enrollment is your annual opportunity to review and update your current benefits for the upcoming year. You can enroll, change or cancel CalPERS health, dental, vision, Flexcash, Dependent Care Reimbursement (DCRA), Health Care Reimbursement (HCRA) or voluntary benefits plans. The effective date for all changes/enrollments made during the Open Enrollment period will be January 1, 2016.

NOTE:  For 2016, the premiums for Anthem Blue Cross Traditional HMO will cost less than the premiums for Blue Shield Access+ HMO.  The level of coverage, benefits and provider network in Humboldt County are the same, however prescription coverage may vary.  If you have questions regarding the coverage of specific medications in a health plan, please refer to the Open Enrollment Benefits Communication (link below) for prescription plan contact information.  Both HMO plans are facilitated through the Independent Physicians Association (IPA) in Eureka; the PPO plans are administered directly by Anthem Blue Cross.

• The health benefits premium rate sheet for 2016.
• All open enrollment forms with original signatures must be received in the Human Resources office, Siemens Hall, Room 212, by 5:00 p.m., Friday, October 9, 2015.
