New Separating Employee Clearance Form

Because of the Covid-19 changes on campus, the process for separating employees has been modified. The new Separating Employee Clearance Form has been updated to reflect such changes.  

As before, there are 3 steps in the process.  Step 1 should be completed by the Department Chair or Director OR the Employee, Department Coordinator or Chair/Director.  The remaining two steps have to be completed by the employee.
