This is a reminder for all Form 200 Filers on Campus. All designated individuals including ProCard holders must file their annual Form 700 no later than April 1, 2022.

Human Resources sent direct email reminders from the eDisclosure system () on March 17, 2022. If you have not filed this year’s Form 700, you should have received a direct email from Form700.CSU@sougthtechhosing.com. If you have not filed your Form700, please check your spam emails or contact Scott Kasper (smk783@humboldt.edu) or Alfredo Corral (ac3599@humboldt.edu) for assistance.

Individual Form 700 filers should have already received an automatically generated access email from Form700.CSU@southtechhosting.com beginning on Monday, February 28, 2022. This email should contain both your log-in ID and a temporary password along with the link to the https://CSU.southtechhosting.com/eDisclosure/ website.

For CPH Form 700 filer’s convenience, please refer to the Quick Guide – Electronically Filing Your Form 700 (also attached below) for filing your Form 700.
