Available Health Plans for 2022

The level of coverage, benefits, and providers for Blue Shield Access+ and Anthem Blue Cross Traditional HMO plans, will remain the same, and must go through the Independent Practice Association (IPA) in Eureka.  The Preferred Provider Organization (PPO) plans are administered directly by Anthem Blue Cross, and Western Health Advantage is administered independently.
Please review the summaries and EOC’s for each plan for complete coverage information about each plan.  You can also use the tools provided on the carrier’s respective websites (linked in the table above) to get more information about local providers.  You may also view the plan comparison chart for a high-level review of plans.

Important Things to Remember/Review:

  • HCRA/DCRA plans DO NOT AUTOMATICALLY ROLL FORWARD – you must re-enroll for 2022 if you would like your enrollment to continue (ASI Flex will send new HCRA enrollees a debit card [two cards will be sent] to be used on reimbursable healthcare expenses, with no additional fee).
  • The Healthcare Reimbursement Account (HCRA) maximum contribution:  $2,750 annual maximum ($229.16/month)
  • Dependent Care Reimbursement Account (DCRA) maximum contribution:  $5,000 annual maximum ($416.66/month)
  • The 2022 Health Premiums Rate sheet.  This is a list of monthly premiums for each health plan.
  • To make changes to your vision plan, or enroll into the Premier plan visit http://csuactives.vspforme.com/, or call customer service at 1-800-  These changes must be completed with VSP directly.  Please note that dependents cannot be split between the Basic and Premier plan.  If you enroll yourself only into Premier (and don’t enroll dependents), your dependents will lose all vision coverage.
  • If you are not making any changes to your plans (including no re-enrollment in HCRA/DCRA), there is no action required.  Please note that if you are currently enrolled on one of the PERS plans, and you don’t select a new plan, your will automatically be enrolled in the respective PERS Platinum/Gold plan.
  • All information regarding Open Enrollment will be available on the Benefits Website.
  • Please print the final page of your enrollment.  eBenefits is not capable at this time to send you a confirmation of your enrollment – please print and save for your records.

Please remember that you have online access to your current benefit enrollment information through your MyHumboldt Portal.  You can review your health enrollments from anywhere, 24/7.  Login to the MyHumboldt Portal, click View Paycheck and Benefits, then View Benefits.
