Annual Conflict of Interest Form 700 Filing for 2021/2022

February 28, 2022

It is that time of year for the annual filing of Statement of Economic Interests (i.e. Form 700) for employees in designated positions and procard users in accordance with the Political Reform Act (CA Gov. Code §81000, et seq.).

The Cal Poly Humboldt Office of Human Resources has implemented a 100% electronic filing system for Form 700’s. This online eDisclosure system eliminates the costs associated with paper filing which included USPS postage, physical paper filing and HR staff time that has been previously and statutorily unavoidable.

What happens next?

Individual Form 700 filers should receive an automatically generated access email from Form700.CSU@southtechhosting.com beginning on Monday, February 28, 2022. This email should contain both your log-in ID and a temporary password along with the link to the https://CSU.southtechhosting.com/eDisclosure/ website.

For CPH Form 700 filer’s convenience, please refer to the Quick Guide – Electronically Filing Your Form 700 (also attached below) for filing your Form 700. You can also visit the Conflict of Interest webpage located on the HSU Human Resource website by clicking this link: Conflict of Interest | Human Resources & Academic Personnel Services (humboldt.edu) where you will find additional tutorials for HSU’s new eDisclosure system. You can also contact Scott Kasper, PsyD, Director of Employee & Labor Relations at scott.kasper@humboldt.edu for compliance questions or Alfredo Corral, Training Specialist at alfredo.corral@humboldt.edu for system questions.

Please note that all Form 700’s must be filed no later than April 1, 2022.
