Action Required: HSU Injury and Illness Prevention Program Questionnaire Complete by 12/13/19

Risk Management and Safety Services(RM&SS) has been endeavoring, this semester, to mitigate deficiencies related to the HSU Injury and Illness Prevention Program (IIPP) that were identified by the CSU Chancellor’s Office Audit and Advisory Services. The audit documented a number of areas where departmental IIPP’s were either deficient or absent altogether, most of which must be corrected by the end of the semester, no later than 12/18/19. 

The Injury and Illness Prevention Program requirement is codified in the California Code of Regulations, Title 8, §3203.  It states, in part: Effective July 1, 1991, every employer shall establish, implement and maintain an effective Injury and Illness Prevention Program (Program). 

The Program shall be in writing and, shall, at a minimum:
1. Identify the person or persons with authority and responsibility for implementing the Program.
2. Include a system for ensuring that employees comply with safe and healthy work practices. 
3. Include procedures for identifying and evaluating workplace hazards… 
4. Provide training and instruction:
--To all new employees;
--To all employees for which training has not previously been received;
--For supervisors to familiarize themselves with the safety and health hazards to which employees under their immediate direction and control may be exposed.

In order to assist RM&SS in the task described above and to close out the audit recommendation, which gets reported to the Trustees and President Jackson, Human Resources (HR) has attached a link to a questionnaire pertaining to particular HSU operations and activities that may trigger specific CalOSHA trainings, to this message.

In order to be reported as complete, department administrators must ensure that each person in their department has responded to the questionnaire to help determine appropriate training content and frequency. Please have the questionnaire filled out by the 12/13/19, so that lesson plans can be developed by the audit correction deadline.
