What’s Included?
Cal Poly Humboldt will fully cover the cost of multiple lab screenings that check for:

• Anemia
• Blood cancers
• Diabetes
• High cholesterol
• Liver and kidney damage
• Thyroid dysfunction
• Blood pressure check
• Body fat percentage screening

WHERE: Nelson Hall East 102 - Goodwin Forum

WHO: Faculty, Staff & Management (currently not available for SPF and students.)  Students have access to a wide range of free or low-cost preventive services through Student Health & Wellbeing Services, including many of the same screenings offered at campus wellness events. Students can learn more or schedule an appointment by calling (707) 826-3146, or by visiting your Health Portal to make certain appointments online.

Fasting is recommended but not required.

You will receive an email after booking with an attached Participant Intake Form along with the Health Screening Informational Sheet outlining the available screening options and requirements on how to prepare for your scheduled appointment. Please print and complete the forms prior to your appointment.
